Ok, I've found the culprit was x360ce joypad emulator, which I use to emulate xbox 360 controller from uHID-G motion controller for nudge... the issue was caused to the fact the controller was not attached a that moment...:p now the game works flawlessly and it's fantastic!
Please can anyone help me? I have a very weird issue: the game runs at 60 fps only if the game window is visible but WITHOUT focus. When the window is FOCUSED (or in full screen with alt-enter) the game is unplayable (about 15-20 fps). Obviously when the game is not in focus it does not receive...

Glad to hear that the portrait mode is not that hard to implement: it's what I'm really missing now from the game. No interest for me about cabinet, just need to use my pivotable monitor at its best.
This is really nice indeed! I wonder if I can achieve to have it properly configured on my pivoted single monitor... I don't know how to extend the window out of screen borders without the help of a second monitor, and more important, I'd lose the DMD :(
Try to launch the game in "windowed fullscreen". On my i7 @4ghz + GTX570 I had similar poor performance. Now is smooth as butter with 16AA, reflection HIGH, VSync ON @1920x1200. Try it and let me know.
I have to say, I tried to launch the game right now in "windowed fullscreen" and it's absolutely smooth... this is weird but... have a try and see by yourself
Thank you. Actually the ball reflection on low setting is almost identical to me, so no problem to disable it. I was wondering anyway, why a game that would seem not that resource demanding, does not run at 60 fps all the time.

with specs as specified in subject, I noticed that, with postprocessing ON, AA 4x, ball reflection max @1920x1200 @fullscreen, when the camera zooms in with some events on the table...
